Well, given how shocked Trixie was that Twilight “changed” Applejack’s gender in the Alicorn Amulet episode, I’d wager they’d probably need a potion for that. Unless there is some spell that’s just really obscure such that most unicorns wouldn’t even have heard about it.
it would make sense that a spell like that would be pretty rare, maybe id imagine the situation depends on what the pony actually has access to like for some potions must be easier but more temporary if u don’t know any unicorns whose magic is strong enough to preform it or even know about it, or maybe in places like canterlot theres unicorns who will charge to cast the spell on you but its about the equivalent to having to go out of state for expensive surgeries which isnt an option for everypony
